• Emergency Coaching Permit Application
• Concussion Education Program (required for emergency and 5-year permit) – Mod 15, Mod 51 (football coaches only) and refresher (Refresher MUST be taken on the 5th yr your coaching cert. is expiring)
• 45-Hour Blended Learning Coaching Course to Obtain a 5-Year Permit (all online modules must be completed online before you attend the last course in person at the CIAC)
• Renew a 5-Year Coaching Permit
• Coaching Modules (CEUs) to Maintain Coaching Certificate (need to take 5 different modules in 5 yrs)
• FINAL Forms Coaches Registration (MUST be filled out once a year prior to working with athletes)
• Print Out a CEU Certificate
• CIAC Website (enter tournament rosters, scores, penalties, pitches, etc.)
• Sexual Harassment Training (print out certificate at end of course and submit to AD)
• DCF Mandated Reporting (print out certificate at end of course and submit to AD)
